Terms of Service

Welcome to Hormone Healer! By accessing or using our website and services, you agree to comply with these Terms of Service. If you do not agree with any part of these terms, please do not use our services.

1. Services Provided

We offer personalized hormone therapy, wellness coaching, functional medicine consultations, and related services designed to optimize your health and wellness. Our services include Bioidentical Hormone Replacement Therapy (BHRT), weight loss programs, and more.

2. Client Responsibilities

By using our services, you agree to:

  • Provide accurate and complete medical information.

  • Follow treatment plans and attend follow-up consultations.

  • Notify us of any changes to your health or medication regimen.

  • Adhere to our telemedicine protocols, including maintaining a secure, private space for consultations.

3. Fees and Payments

Fees are due at the time of service. The following services and their respective fees are outlined:

  • Initial Consultation (1 hour): One Time 60-minute Hormone Consultation: $500 includes medical history review, lab results review, and treatment recommendations (lab work cost not included; patients may complete labs independently or through our office).

  • Follow-Up Appointments:

    • 30-minute follow-up: $150

    • 45-minute follow-up: $225

    • 60-minute follow-up: $275

  • Communication Fees: Emails and phone calls exceeding 10 minutes: $150 (billed at the 30-minute follow-up rate).

All fees are due prior or at the time of service unless otherwise arranged.

 

4. Confidentiality

We adhere to HIPAA regulations to protect your privacy. All information provided to us will be kept confidential unless otherwise required by law or for treatment purposes. This includes telemedicine consultations, where we will ensure the security of your data.

5. Medical Disclaimer

Our services are not a substitute for primary care or emergency medical care. Hormone therapy and wellness services are intended to optimize hormone health and overall wellness, but they are not meant for diagnosing, treating, or curing medical conditions outside the scope of hormone therapy. Always consult with a physician before making changes to your health regimen.

6. Cancellation Policy

  • Appointments canceled with less than 24 hours' notice or missed without prior notice may incur a fee equal to the cost of the scheduled appointment.

  • Rescheduling is possible with at least 24 hours' notice.

7. Liability

Hormone Healer is not responsible for any indirect or consequential damages resulting from the use of our services. You are responsible for your health decisions and actions.

8. Telemedicine

Telemedicine consultations are available, allowing you to receive care from the comfort of your home. You understand that the telemedicine session may involve video conferencing or phone calls, and there may be technical limitations. If necessary, either party may choose to discontinue the telehealth visit if the connection is inadequate. Telemedicine services are not an emergency service, and in case of an emergency, you should immediately seek care from a local healthcare provider.

9. Termination of Services

You may discontinue services at any time but will remain responsible for any outstanding fees. We reserve the right to terminate this Agreement if:

  • We determine that the Practice and Patient are not a good fit.

  • You fail to adhere to our payment or treatment policies.
